2. Annual Report Documentation Form: This form can be used each fiscal year for recording details of each item and, thus, for providing backup documentation for individual faculty.
3. Faculty Coding Sheet: This form is for reporting summarized counts of each item (items 1-97) by individual faculty. Use either the Excel or the Word form.
4. Department Summary Template: This template is provided as an optional tool to assist the departmental administrative assistant and department head to tally the individual faculty responses to items 1-97. The tallied items can then be reported on the Department Head Summary Sheet (see 5.).
5. Department Head Summary Sheet: This form is for reporting the tallied individual faculty responses to items 1-97. Responses to items 98-104 are to be provided by the department when available. This form, when completed, should be submitted by the department as part of the annual report process. An electronic copy of the form can be submitted to OIR. Use either the Excel or the Word form.
